Lance Corporal Anastasio was taking part in Operation Meade River, approximately 7 kilomters S-SE of Dien Ban, Quang Nam Provinnce, when the LVTP-5 B-38 Tractor he was driving detonated a mine. He died as a result of the burns to his body from the explosion. .

LCpl Anastaqsio was survived by his wife, Pamela E. and daughter Joi M who were living in Baltimore at the time. He was also survived by his father Mr. Joseph B. Anastasio and Mrs. Henry N. Anastasio, also of Baltimore. LCpl Anastasio was buried in Moreland Memorial Cemetery, Baltimore, Maryland.
