Four men of the 162nd Assault Helicopter Company died when their UH-1H (tail number 69-15429) was hit, crashed, and exploded while they were coming into a hot landing zone for a combat extraction:
  • WO William H. Laurence, San Antonio, TX, pilot
  • CPT Stephen D. Carr, Barre, VT, copilot
  • SP5 Benny R. Halstead, Glen Morgan, WV, gunner
  • SP5 Larry B. Jacobson, Norma, ND, crew chief
