A Note from The Virtual Wall

Although a member of Headquarters Battery, 1st Battalion, 77th Arty, Sergeant Carl R. Ussery was attached to HHC, 1st Battalion, 5th Cavalry, as a Forward Observer.

According to the VHPA helicopter incident database, Sergeant Ussery was aboard a resupply flight in a UH-1H (hull number 67-17468) of C Company, 227th Avn Bn, which was shuttling between LZ Nancy and a small tactical LZ, resupplying a ground unit in heavy contact with the enemy. On the third shuttle, the helo contained the four crewmen, a cargo of boxed 82mm mortar ammunition (much of which was white phosphorus), and four passengers going to the unit being resupplied. The passengers were seated on and around the cargo, without seat belts or any form of restraint.

On this approach, at an altitude of perhaps 20 feet, the main rotor struck a large limb near the top of a tall tree, at about the 4 o'clock position to the helicopter. A ground guide was directing the aircraft, and did not see the tree. The aircraft rolled to the right, impacted the ground, and began to burn. Although all four crewmen and two passengers were able to escape the aircraft uninjured, the two remaining passengers (SGT Carl Ussery and CPL Raymond Evans, B/1/5th Cav) died in the accident.
