The 1/1 Marines Command Chronology and 1st Marines Operations Journal for August 1967 set forth the circumstances:
"271625H - Spot Rpt #14 Co B rcvd 5 KIA and 10 WIA when plt accidentally entered minefield."

"Company B patrol unknowingly entered an ARVN minefield at BT 169574 resulting in 5 killed and 10 wounded from the unit and reaction force sent to assist them."
