A Note from The Virtual WallOn 06 May 1970 the 158th AHB was tasked with laying a smoke screen on a landing zone near enemy positions in Quang Tri Province. During the operation, the skids of one helicopter (66-16707) impacted the rotor disk of a second (68-15663), causing both aircraft to crash. While the gunner from 68-15663 survived and was able to report the cause of the incident, seven men died:
