Notes from The Virtual Wall C Company, 228th Assault Support Helicopter Battalion, lost five men when their CH-47A (tail number 63-07910) suffered a mechanical failure and broke up in flight: CWO Billy J. Waymire, Little Rock, AR, pilot CWO James W. Leach, Sanatorium, MS, copilot SP5 Raymond E. Smoot, Boise City, OK, gunner SP4 Carl Brown, Northport, AL, gunner PFC Michael G. Isely, Slidell, LA, crew chief Years after his death SP5 Smoot's wife received his posthumous award of the Purple Heart, but it is unclear if the award resulted from an Army review of the loss of CH-47A tail number 63-07910 (i.e., a determination that it was a combat rather than an operational loss) or from injuries received on another occasion. SP5 Smoot also received the Bronze Star, Air Medal (7 OLCs), and the Republic of Vietnam's Military Merit Medal and Cross of Gallantry with Palm (individual award).
